Description:JOB SUMMARY/DESCRIPTION:
Functions as a specialist in transition care providing professional care to patients with special health care needs. Facilitates patient/family access to health care systems. Cares for patients by assessing needs, creating and implementing transition plans, providing ongoing surveillance and care coordination throughout transition process, and evaluating outcomes. Provides feedback regarding Transition Support program outcome and statistics to the CYACC Medical Director and Program Manager. Works in clinical setting 40% of time and administrative setting 60% of time; general hours of Monday – Friday 8:30am - 5pm with need for flexibility.
Duties:ESSENTIAL FUNCTIONS:
• Interacting with all internal and external customers in a caring and respectful manner in accordance with IUMG Core Values philosophy.
• Functions as both registered nurse and care coordinator for assigned patient load.
• Specific nursing tasks include scheduling consults, calling medications, completing medical prior authorizations, informing regarding test results, triaging medical concerns, assisting with new PCP introduction of IHAP, supporting team members needs regarding medical interventions.
• Works as part of an interdisciplinary team of health professionals in caring for youth and young adults with special health care needs and adults with developmental disabilities.
• Provides information concerning clinical practice for Transition Support patients to other providers.
• Coordinates contacts with patients, families, health care, social service and community service agencies to provide/obtain Transition Support services.
• Provides coverage and education for other interdisciplinary Transition Support team members as needed and actively participates in cross training and team building with other members of the interdisciplinary team.
• Performs other job-related duties as assigned.
• Maintains protected health information in accordance with HIPAA privacy guidelines.
Qualifications:KNOWLEDGE, SKILLS, AND ABILITIES:
• Knowledge of, or ability to learn, change and other relevant theories, current clinical concepts including nursing theories and research; normal behavior and development of youth and young adults; treatment modalities and resources appropriate for youth with special health care needs.
• Ability to communicate effectively
• Ability to self direct ongoing professional education
• Ability to demonstrate independent decision making
• Ability to be flexible
• Ability to effectively work under stressful conditions

MINIMUM QUALIFICATIONS, TRAINING, AND EXPERIENCE:
• Current Indiana License as Registered Nurse.
• Clinical experience in pediatric and/or adolescent health care with experience/interest in caring for youth and/or adults with special health care needs.
• Current driver’s license and proof of insurance when role includes home or community visits.
• Experience in working with community agencies in a collaborative model is a definite plus.
• Experience with group or workshop presentations is also desirable.
